Summer Mime Workshop Application Checklist
1. Application and Questionnaire, Agreement - Download, fill out, and return the Word Version. The document includes a copy of the Participant Agreement, which much be signed and returned with the application.
2. Picture - attach when you submit the online application and questionnaire, or e-mail or mail in to LCDS.
3. Statement of Motivation and Intention - attach when you submit the online application and questionnaire, or e-mail or mail in to LCDS. The statement is self-designed and should reflection your motivations and intentions for attending the workshop. It can be submitted as a word document or a hand-written document.
4. Payment - A deposit of $250 is required to reserve your space. Full payment of $895 is due by June 10. After you have submitted all of your application materials and received an acceptance e-mail you will need to submit your payment by mailing a check or paying online via paypal. Your acceptance e-mail will include a link to the payment information page.
5. Congratulations, you're done. We look forward to seeing you at the summer workshop!
